Enrollment Package

The enrollment package is available here for you to download. Please download and print it out. Then fill it out and turn it in to Jeannie Silvia at Choice 2000 either via fax at 951.940.5706 or by bringing it to Choice 2000 directly. Mrs. Silvia will be happy to help you with any questions about the enrollment package.




The enrollment package is in Adobe PDF format and you will need Acrobat Reader on your computer to view it properly. Most computers have Acrobat Reader already. If you do not have Acrobat Reader, you can download it here.
